Generating a New CMS-1500 Form

To generate a new CMS-1500 Form:
  1. Go to the claim and click Lightning Bill to add the claim to the Claims Queue.
  2. Navigate to the Claims Queue.
  3. On the relevant Superbill, click the Print button.

Printing the Most Recently Submitted CMS-1500 Form

If you need to view or print the CMS-1500 form that was most recently billed:
  1. Open the specific claim in Elation Billing.
  2. Click the Print button located at the top of the screen.
  3. The form will reflect the version that was most recently billed and is ready for printing.
