How do I share a Care Plan with my patient via Patient Passport?
If a Care Plan was documented in a visit note, then the Care Plan will automatically be shared with the patient via Patient Passport in the form of a Visit Summary once the visit note is signed off. There are a couple of ways you can search for patient’s who are due for an annual wellness exam:- Use the Patient List report and search for patients using the Last Seen and CPT Codes filters. For example, apply the Last Seen filter and search for all patients who had their last visit in 2024, then use the CPT Code filter and filter by the CPT codes for annual wellness visits (i.e. 99395, 99396, G0439, etc).
- Use Billing Home and search for visits that had annual wellness visit CPT codes and then calculate when the patient is due for their next annual. For example, set the date range for the enter 2024 calendar year and then download the results and filter by the CPT codes for annual wellness visits (i.e. 99395, 99396, G0439, etc).

How do I enter the patient’s height and weight in metric units?
A patient’s height and weight can be entered in metric units but it will be converted to and stored as imperial units.- To enter the patient’s height in metric units, type the number and then ‘cm’ (i.e. 165cm). The height will then be converted to inches.
- To enter the patient’s weight in metric units, typer the number and then ‘kg’ (i.e. 65kg). The weight will then be converted to pounds.
How do I record historical vitals?
Use a Non-Visit Note to store historical vitals by following these steps:- Click Visit Note at the top of the patient’s chart.
- Click Non-Visit Note.
- Click Vitals to open the Document Vitals box.
- Enter vitals information as needed for the patient and make sure you update the date of the vitals records.
- Click Save to save and close the box or Save and Add Another to keep adding more vitals.
- Click Sign Note to save the Non-Visit Note and corresponding historical vitals.
