The Patient Passport invitation workflow and patient-facing web portal experience has recently been updated. Look in the following articles for more detailed information:

Overview

What is Patient Passport?

Patient Passport is a secure online portal that gives your patients 24/7 access to health information you’ve shared. It also allows you and your patients to send secure messages to each other. Patient Passport is accessible via any web browser.
The current Elation Passport iOS app reflects the previous Patient Passport experience and will not change with this update. A new native app that matches the updated web experience is coming soon. For the latest experience, patients can use Patient Passport at app.elationpassport.com from any browser on their phone or tablet.

Who can use Patient Passport?

Patients and/or their family members and friends — such as parents, guardians, or caregivers — can use Patient Passport to access a patient’s health information as long as they have an email address they can use for registration.

What are the benefits of Patient Passport?

With Patient Passport, you can:
  • Make it easy for patients or their family members and friends to access health information anytime, anywhere — through any web browser.
  • Improve communication through secure, non-urgent messaging.
  • Support care coordination by enabling patients to share their records.
It’s a great way to enhance patient engagement and streamline communication.

What patients can see by default in Passport (for practices)

Patients will automatically have access to the following once they have an active Passport account:
  • Patient Letter messages and attachments you explicitly send
    • Patients can receive written messages and attachments from your practice.
      • Settings are available to control whether patients can respond to your message and start their own message threads.
  • Demographics
    • Patients have real-time access to the following demographics Information from their chart:
      • Date of birth
      • Emergency Contact
      • Provider Assigned in Practice
  • Clinical Profile
    • Patients have real-time access to the following Clinical Profile details from their chart:
      • Allergies
      • Drug Intolerances
      • Medications
      • Vaccines
    • Settings are available to share more from the Clinical Profile.
  • Visit summaries for all signed visit notes
    • Patients will automatically receive generated summaries when you sign a visit note. This includes the information documented in following sections of signed visit notes:
      • Vitals
      • Proc (Procedures)
      • Tests/Treatments
        • This includes any Referrals, medications and diagnostic test orders.
      • Care Plan (Instructions)
      • F/U (Follow-up)
  • Appointment details

What is not shared automatically

  • Visit notes in Requiring Action or Outstanding Items – Visit summaries cannot be shared until they are signed off.
  • Confidential records – Notes in the Confidential section of the Clinical Profile are never shared via Passport.
  • Other chart documents – Lab reports, imaging results, and other records are only shared if you explicitly attach them to a Patient Letter.

Clinical Profile sections and default sharing settings

Items in the patient’s Clinical Profile can also be shared automatically; the specific sections and amount of information depend on the chosen sharing options:
  • Objective Data Only
  • Satisfies MIPS Requirements
  • Entire Clinical Profile except the Confidential section
  • Entire Clinical Profile, Expanded Summary, excluding Confidential
Passport sharing settings can be customized for each patient within the Passport dialog (globe icon) in their chart. The default sharing behavior is set via Patient Sharing Defaults in a provider’s User Settings. These defaults apply to patients who have that provider listed as their Provider Assigned in Practice under their demographics. Any updates to the default settings will only affect newly invited patients. Click here for more details and instructions for customizing default sharing settings.

Frequently Asked Questions

Can I customize Passport access on a per-patient basis?

Yes. Sharing options can be overwritten on a per-patient basis when Patient Sharing Defaults do not suffice. You can:
  • Override Clinical Profile sharing per patient – Click the Passport button (globe icon) in the patient’s Clinical Profile and select a different option under Passport Sharing Options.
  • Choose what you send to each patient – You decide which letters, lab results, or other records to attach to Patient Letters for individual patients.
The behavior for visit summaries and default Clinical Profile sharing remains global per provider. Changes to your sharing defaults only apply to patients who register for Passport after the update; for patients already registered, adjust sharing settings individually in their chart.
