We’ve updated Elation Patient Passport with a new look and new features. If you’ve used Patient Passport before, your login still works. You do not need to create a new account.
Works on any devicePatient Passport works on any web browser, including on phones and tablets.The Elation Passport app is also available on iOS and Android devices.
Faster loginSetting up your account takes less than two minutes. If you download the Elation Passport app, you can use Face ID or Touch ID on iPhone, or fingerprint authentication on Android, instead of typing your password each time.A simpler home screenWhen you log in, your appointments, messages, and health records are right there on the home screen. Everything is easier to find.Upcoming and past appointmentsYou can see all of your upcoming and past appointments in one place. Depending on your provider’s settings, you may also be able to cancel an upcoming appointment from Patient Passport.One login for multiple family membersYou can access more than one patient chart with a single login. This is helpful for parents, guardians, and caregivers who manage health records for a family member. Use the Switch patient view menu in the top right corner to move between charts. Your provider’s office must set this up for you.Confirm who you are on first loginThe first time you log in, you may be asked to confirm your relationship to a patient chart. You will see three options:
This is me — the chart belongs to you
I manage their care — you are a parent, guardian, or caregiver for this patient
Remove this chart — you do not recognize it or do not want access
Language selectionYou can choose your preferred language in Patient Passport. English, Spanish, Chinese, Japanese, Korean, Tagalog, and Vietnamese are supported.Only Passport’s interface (menus, buttons, and labels) is translated. Messages, visit summaries, letters, and any documents your care team sends you appear in the language your provider wrote or uploaded them in.Visit summariesAfter each visit, your provider signs a note and a summary is added to Patient Passport. You can see a list of all your past visit summaries and read the details of each one.Health records and documentsYou can view and download your health summary, medications, allergies, and vaccines. Any records your provider shares with you, such as lab results or forms, are also here.Download your immunization historyYou can download and print your immunization history as a standalone document from Patient Profile → Share my records. This is especially useful for school enrollment forms, sports physicals, or any time you need to share just your vaccine records.Send attachments in messagesYou can attach files to messages you send to your care team, such as photos or forms. Depending on your provider’s settings, you may also be able to start a new message from Patient Passport.You’re in control of your namePassport now lets you set your own Account Name, completely independent of any legal name or chosen name on file. This is the name the care team sees when you message them. Previously, the Account Name was automatically tied to the patient’s legal name, which wasn’t always the name you actually go by. Now, you decide — the first time you log in, we’ll ask you to confirm the name you’d like to use, and you can change it anytime in Account Settings.Passport also keeps your Account Name separate from the patient’s chosen name on each chart you can access. The person signed in and the patient whose chart is open may not be the same person (for example, a parent managing care for their child). You can update a patient’s chosen name from their Profile under Patient Demographics > Patient Details > Full Name (if different from legal). This change flows back to the practice.Update your account informationYou can update your password, email address, and phone number from Patient Passport. Select Account Settings in the top right corner of the screen.Update your contact detailsIn the Demographics section, you can update the patient’s chosen name (the “Full Name (if different from legal)” field), pronouns, gender identity, contact information, and emergency contact. Any changes you make will show up in your chart right away. Your legal name, date of birth, and sex at birth can only be updated by your provider’s office.
The patient’s chosen name is now used across more patient-facing surfaces, including appointment confirmation texts and calls, cancellation emails, the SMS opt-in page, booking confirmations, and the patient profile print view. Insurance, lab, pharmacy, and other compliance-sensitive workflows continue to use your legal name.
Easier sign-upCreating an account takes less than two minutes. Instead of receiving a code by text message, you will confirm your identity using your date of birth.New invitation emailWhen your provider invites you to Patient Passport, you will get a new-look email from Elation. Just select the link in the email to get started.Access for family members and caregiversYour provider’s office can invite a parent, guardian, or caregiver to view your chart. Contact your provider’s office to request access for a family member or caregiver.
