# Elation Patient Passport Guide - Patient's Point of View

> This article displays the patient's experience using Elation Patient Passport, including the registration process and messaging functionalities.

## Overview

### How do I get a Passport account?

To get a Passport account, you first need an invitation from your doctor’s office.

### How do I access my Passport account?

You access your Patient Passport account 24/7 via any web browser or the [Elation Passport iOS app](https://apps.apple.com/us/app/elation-passport-for-patients/id1264836684).

## Setup

### Registering for a new account

| **1**  | Login to your email account and look for the most recent Passport invitation email. It is from the email domain " [http://elationpassport.com/](http://elationpassport.com/) " with one of the following subject lines: Invitation to Elation Passport - Your Secure Patient Portal • \[Doctor's Name] has a secure message for you |
| ------ | ----------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------- |
| **2**  | Open the email and click **Click here to Register for Passport** and a new page will open up.                                                                                                                                                                                                                                       |
| **3**  | If your email is text based, copy and paste the long URL that you see at the center of the email body into your web browser.                                                                                                                                                                                                        |
| **4**  | Click the link to the 2-minute video for a visual walkthrough of the registration process if needed.                                                                                                                                                                                                                                |
| **5**  | Enter a unique password twice.                                                                                                                                                                                                                                                                                                      |
| **6**  | Enter your 7-digit Passport Invitation Code. This code is typically texted to the cell phone number in your chart. If your doctor's office gave you an instruction sheet, you can also find the code there.                                                                                                                         |
| **7**  | Review the terms of service and then check **I agree to Elation's Terms of Service** if you agree to the terms.                                                                                                                                                                                                                     |
| **8**  | Specify your relationship to the account: **I am the patient:** Check this box if the Passport account is for you. • **I am the legal representative of the patient:** Check this box if the account is for your child, a family member, or anyone else under your care.                                                            |
| **9**  | Click **Submit** to complete registration.                                                                                                                                                                                                                                                                                          |
| **10** | You will then be taken to your Passport account.                                                                                                                                                                                                                                                                                    |

After successfully registering for your Passport account, you will receive an email confirming your registration. This email also provides you with instructions on how to access your Passport account again in the future.

### Linking a new Passport account to your existing Passport account

| **1** | If you have multiple providers who use Elation, you can link your Passport accounts together under the same email address.                                                                                                                                                                                                          |
| ----- | ----------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------- |
| **2** | Login to your email account and look for the most recent Passport invitation email. It is from the email domain " [http://elationpassport.com/](http://elationpassport.com/) " with one of the following subject lines: Invitation to Elation Passport - Your Secure Patient Portal • \[Doctor's Name] has a secure message for you |
| **3** | Open the email and click **Claim Your Chart** and a new page will open up.                                                                                                                                                                                                                                                          |
| **4** | If your email is text based, copy and paste the long URL that you see at the center of the email body into your web browser.                                                                                                                                                                                                        |
| **5** | Login to your existing Passport account to verify your identity.                                                                                                                                                                                                                                                                    |
| **6** | Enter the date of birth of the person whose account you’re claiming in MM/DD/YYYY format (slashes included). This could be your own date of birth if you’re linking multiple accounts from different providers.                                                                                                                     |
| **7** | If you are not the patient, check off **I am the legal representative of this patient** .                                                                                                                                                                                                                                           |
| **8** | Click **Continue** .                                                                                                                                                                                                                                                                                                                |
| **9** | A **Congrats** page will appear to confirm you’ve successfully linked Passport accounts. Click **Check It Out** to proceed to your account.                                                                                                                                                                                         |

## Workflow Instructions

### Using your Passport account from a web browser

#### Logging in to your Passport account from a web browser

| **1** | You can access your Passport account 24/7.                               |
| ----- | ------------------------------------------------------------------------ |
| **2** | Go to [http://app.elationpassport.com/](http://app.elationpassport.com/) |
| **3** | Enter your email address and password.                                   |
| **4** | Click **Login** .                                                        |

#### Switching between linked accounts

If there are multiple Passport accounts linked to your account, switch to a different account profile by following these instructions:

| **1** | Click on the current profile name at the top right of the account. |
| ----- | ------------------------------------------------------------------ |
| **2** | Click on the name of the profile you want to switch to.            |

<Note>
  You can only switch between linked accounts while logged in to Passport from a web browser, not from the iOS app.
</Note>

#### Managing your account settings

To adjust your account settings, click on your name at the top of the account and then click **Settings**. Here you can adjust the following:

* **Set preferred contact method**
  * Select either Email or SMS Text message as your preferred contact method.
* **Login and Security**
  * Change your login email by clicking **Change Email**.
    * To change your email address, you'll first receive an email asking you to verify your new one.
  * Change your login password by clicking **Change Password**.
* **View Passport Usage Log**
  * View all login attempts for the account.

#### Viewing your Timeline of messages and visit summaries

When you log in, you'll see your Timeline by default.

* If you have any unread messages or visit summaries, you'll see the **New** section.
* If there are no new messages or summaries, you'll see the **All** section.

#### Viewing your Clinical Profile

Click into the Clinical Profile section to see what is stored in the Clinical Profile section of your chart.

* Click **Download My Data** to get a copy of your Clinical Profile data in CCDA format. You can either download it to your device or email it.
* Click **Fax Profile** to fax a copy of your Clinical Profile.
* Click **Print Profile** to print a copy of your Clinical Profile.

#### Messaging your doctor’s office

<Note>
  If your doctor’s office has disabled Passport communications then you will not be able to send new messages or reply to existing messages.
</Note>

##### Sending a new message

| **1** | Click the **New Message** button at the bottom of your account.                        |
| ----- | -------------------------------------------------------------------------------------- |
| **2** | Select a category for the **Subject** of your message.                                 |
| **3** | Type your message in the **Message** box.                                              |
| **4** | Click **Add Attachment** to attach records to the message as needed.                   |
| **5** | Select the record(s) that you want to attach from your computer.                       |
| **6** | Click **Open** .                                                                       |
| **7** | Click **Send**. You can find the sent message in the **All** section of your Timeline. |

##### Replying to existing messages

| **1** | Click on the message you want to reply to from your **Timeline** . |
| ----- | ------------------------------------------------------------------ |
| **2** | Type your response in the **Reply to your doctor’s office** box.   |
| **3** | Click **Reply** .                                                  |

<Note>
  You cannot add attachments to existing messages. To send attachments to your doctor’s office, create a new message.

#### Booking an appointment via your doctor’s Booking Site

If your doctor’s office offers self-booking appointments, click **Timeline**->**Book appointment** to book an appointment with them.

### Using the Elation Passport iOS application

#### Logging in to your Passport account from the Elation Passport iOS application

You can access your Passport account 24/7.

| **1** | Open your [Elation Passport iOS application](https://apps.apple.com/us/app/elation-passport-for-patients/id1264836684) . |
| ----- | ------------------------------------------------------------------------------------------------------------------------ |
| **2** | Enter your **email address** and **password** .                                                                          |
| **3** | Press **Login** .                                                                                                        |

#### Switching between linked accounts (iOS)

You can only switch between linked accounts while logged in to Passport from a web browser.

#### Managing your account settings (iOS)

To adjust your account settings, press **Settings** at the bottom of the App. Here you can adjust the following:

* **Notification**
  * Select your preferred contact method - **Email** or **Text Message**.
  * Enable/disable **Push Notifications**.
* **Login and Security**
  * Enable/disable the **Remember Me** feature.
  * Change your login email.
    * To change your email address, you'll first receive an email asking you to verify your new one.
  * Change your login password.
* **Audit Log**
  * View your Passport usage log.

#### Viewing your Inbox of messages and visit summaries (iOS)

When you log in, you'll see your **Inbox** by default.

* If you have any unread messages or visit summaries, you'll see the **Unread** section.
* If there are no new messages or summaries, you'll see the **All** section.

  To print your records, login to your Passport account via a web browser.

#### Viewing your Clinical Profile (iOS)

Press **Profile** to see what is stored in the Clinical Profile section of your chart.

You can also press the export icon and then **Share via Fax** to fax a copy of your Clinical Profile.

<Note>
  To print your Clinical Profile, login to your Passport account via a web browser.
</Note>

#### Messaging your doctor’s office (iOS)

<Note>
  If your doctor’s office has disabled Passport communications then you will **not** be able to send new messages or reply to existing messages.
</Note>

##### Sending a new message

| **1** | Got to your **Inbox** .                                                                                                                                                                            |
| ----- | -------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------- |
| **2** | Press the **New Message icon** at the top of the inbox.                                                                                                                                            |
| **3** | Press **Select a reason…** to indicate the reason for your message.                                                                                                                                |
| **4** | Type your message where it says **Write your message here** .                                                                                                                                      |
| **5** | Press the **Add Attachment icon** to attach records to the message as needed. Select the record(s) that you want to attach from your device by pressing **Take Photo** or **Choose From Library**. |
| **6** | Press **Send** .                                                                                                                                                                                   |

You can find the sent message in the **All** section of your Timeline.

##### Replying to existing messages

| **1** | Press on the message you want to reply to from your **Inbox** .  |
| ----- | ---------------------------------------------------------------- |
| **2** | Type your response in the **Reply to your doctor’s office** box. |
| **3** | Press **Reply** .                                                |

<Note>
  You cannot add attachments to existing messages. To send attachments to your doctor’s office, create a new message.

#### Booking an appointment via your doctor’s Booking Site (iOS)

If your doctor’s office offers self-booking appointments, click **Booking**->**Book appointment with Elation Demo Practice** to book an appointment with them.

### Receiving account notifications

Each time something new is sent to your Passport account, be it visit summaries or messages, you will receive a notification via your preferred contact method. Simply login to view the new items in your **Timeline**/**Inbox**.

* SMS Text Message reminders will come from the number 36331 - we recommend saving this number in your Phone book or Contacts list so that it is not flagged as Spam.
