# Change Provider Guide - Bulk reassigning patients to a new provider

> The Change Provider bulk action lets you reassign multiple patients to a new primary provider at once from the Patient List Report.

## Overview

### What is the Change Provider bulk action?

The **Change Provider** bulk action lets you reassign a group of patients to a new primary provider in one step, instead of updating each patient's chart individually. It's available from the **Patient List Report**, using the same multi-select workflow as [Send Bulk Letter](/articles/introduction-to-bulk-letters) and [Change Status](/articles/Patient-Status).

### What can I use it for?

* A provider is leaving the practice and their patients need a new primary provider — see [Patient Passport Messages](/articles/User-Accounts-Guide-Best-practices-for-deactivating-accounts#patient-passport-messages) in the deactivation guide.
* Rebalancing patient panels across providers.
* Any other bulk change to who's listed as a patient's **Provider assigned in practice**.

## Workflow Instructions

### Bulk reassigning patients

<Steps>
  <Step title="Generate a Patient List">
    Go to **Reports** → **Patient List** and filter for the patients you want to reassign (e.g. by current provider).
  </Step>

  <Step title="Select the patients">
    Check the box next to each patient, or check the box at the top of the list to select every patient matching your filters — not just the current page.
  </Step>

  <Step title="Open Change Provider">
    Click the **Edit Patients** dropdown at the top of the results and select **Change Provider**.
  </Step>

  <Step title="Choose the new provider">
    Search for and select the **New primary provider** in the dialog.
  </Step>

  <Step title="Review what will and won't change">
    The dialog lists what updates automatically and what you'll still need to handle yourself (see below). If any selected patients are VIP, a note tells you how many will be skipped.
  </Step>

  <Step title="Click Reassign">
    Click **Reassign \[N] patients**. Reassignment runs in the background — you'll see a confirmation toast when it's done. If the new assignment doesn't appear right away, click **Generate List** again to refresh the report.

### What changes automatically

* The **Provider assigned in practice** field on each patient's Demographics record.
* New Patient Passport messages start routing to the new provider going forward.

### What you still need to handle yourself

Reassigning a patient's provider does **not** move the following — handle each of these individually:

* **Prescription refills** — the new provider needs to send a fresh prescription to the pharmacy; existing refill requests stay tied to the original prescriber.
* **Message threads** — past Patient Letters and Office Messages stay in the old provider's inbox.
* **Scheduled appointments** — [edit each appointment](/articles/User-Accounts-Guide-Best-practices-for-deactivating-accounts#appointments-with-the-departing-provider) to move it to the new provider's calendar.
* **Pending refill requests and unsigned lab orders** — these need manual, per-patient review by an active provider.
* **Care team memberships** — the old provider isn't automatically removed from the [Care Team](/articles/the-collaborative-view-hi-only).

### VIP patients

[VIP-marked patients](/articles/vip-chart-feature) are only reassigned automatically if the person running **Change Provider** is already that patient's assigned provider. Otherwise, the VIP patient is skipped — the dialog tells you how many were skipped and why.

To reassign a skipped VIP patient, either have their current provider run the bulk action, or [un-mark the VIP designation](/articles/vip-chart-feature#un-marking-charts-as-vip) first (only the **Provider assigned in practice** can do this) and reassign the patient individually via [Patient Demographics](/articles/Patient-Demographics-Guide#provider-information).

## Frequently Asked Questions

### How many patients can I reassign at once?

Up to 5,000 patients per reassignment. If you need to reassign more, split the list into multiple batches.

### How long does it take?

Reassignment runs in the background and can take a little time for large selections. You'll see a confirmation toast when it finishes; refresh the Patient List Report (**Generate List**) if you don't see the updated provider right away.

### Can I undo a bulk reassignment?

There's no dedicated undo. To reverse it, select the same patients and run **Change Provider** again with the original provider.

### Does this cancel or move appointments?

No. Scheduled appointments stay on the original provider's calendar — see [What you still need to handle yourself](#what-you-still-need-to-handle-yourself) above.
